Mo HealthNet Proposes Rule Expanding Health Home Conditions
Mo HealthNet recently published a proposed rule expanding the conditions that make patients eligible for Primary Care Health Homes.
The proposed rule adds obesity and childhood uncontrolled asthma as standalone conditions that qualify Medicaid patients for Primary Care Health Homes. It also adds anxiety, depression, and substance abuse to the list of conditions that, in conjunction with other conditions, can qualify someone as a PCHH patient. DSS estimates this change will add 2,500 patients to the health home program.
The rule also requires PCHH teams to include to a "physician champion" to encourage transformation to the health home model.
