Chronic Care Management Webinar

As of January 1, rural health clinics may receive additional payment for the costs of chronic care management (CCM) services provided to certain Medicare beneficiaries. Eligible patients must have two or more serious chronic conditions.

If your clinic has questions about CCM services, you are invited to join the National Association of Rural Health Clinics (NARHC) for a technical assistance webinar. Speakers from CMS will present on service delivery requirements, signing up patients, and billing for CCM.
